Opinion: Soldier accused of shooting rampage: Not PTSD alone

The shocking news of an American soldier allegedly going on a shooting rampage and taking the lives of 16 Afghanistan civilians has captivated the world this week. The question everyone is trying to answer: What caused him to snap and commit such a heinous act?

Details are slowly emerging, but as a former Army doctor and someone who has evaluated more than 7,000 veterans with PTSD (Post Traumatic Stress Disorder), I believe a combination of factors pushed the unnamed soldier over the edge.

This soldier was more than likely suffering from PTSD and Traumatic Brain Injury (TBI), but these conditions alone would not have caused him to go on a killing spree. Even in the most severe cases I’ve seen in my 30 years of research and treating patients, I have never seen or heard of anyone with PTSD alone doing such a thing. Yes, PTSD typically brings on symptoms of anger, irritability, and even at times, rage.  It causes a person to feel distant and detached, easily startled or hyper-vigilant. The person might be unsociable and have trouble expressing his or her emotions. But killing 16 people is not a result of combat-related PTSD alone.   

In addition to suffering with PTSD, something else is the primary reason he went out of control.  Possibly he was suffering from drug or alcohol abuse, major depression, psychosis or other conditions. He might have also had an argument with a fellow soldier or with his wife back home. Or perhaps he received some upsetting news that made him snap.

Reports also suggest that the day before the incident, the unnamed soldier witnessed a horrific event: someone close to him, possibly another soldier, becoming seriously injured. Some reports say this fellow soldier’s leg was shot off.  We don’t yet know the relationship to this injured person, but if it was someone fairly close, it could have pushed the assailant further into a depressive state.

What most non-military people don’t realize is just how strong the relationships are between fellow soldiers. Military “brothers and sisters” are constantly in life and death situations and are always watching each other’s backs. That’s why when we see soldiers with a psychiatric condition, it’s not uncommon for them to cope better with their military friends and “family” than their real family back home.  The stress these men and women are under on a daily basis is unimaginable to the average person.  And this war has added another factor: repeated deployments to the same combat areas, something unprecedented in the history of modern American warfare.

In fact, for many soldiers now serving, especially those with PTSD, even when home they are never really totally home. They are always thinking about the combat zone and what their next deployment will bring.  In the case of the unnamed soldier, we now know he didn’t want to be deployed again. This was his fourth deployment and reports are he didn’t want to go back and that doing so was a last-minute event. If he was suffering from depression, this could have potentially made that a lot worse. If he was using drugs and alcohol to cope with his PTSD, being redeployed could have made him start using even more.

The other interesting piece to the puzzle is that immediately after the killings, the accused soldier didn’t run or hide. He came back to base and turned himself in. This tells us he clearly knew what he was doing, which makes an insanity defense unlikely.

A lot of attention has also focused on this soldier’s TBI (traumatic brain injury). He apparently lost part of a foot and suffered a concussion on a past deployment. Psychiatrically speaking, although a concussion or TBI can loosen conscious control over behavior, it is unlikely that both conditions occurring in the same troop would cause something like this to happen. There had to be multiple other factors that came into play.

Some people have said perhaps where he was based played a role. This soldier was stationed at Joint Base Lewis-McChord, which houses a unit that has been under tremendous stress and saw 12 suicides this past year alone. It also has a high divorce rate among soldiers who live there. Despite any of this, it’s still not enough of an explanation - by itself - of why someone would do this.

In the coming weeks and months we will certainly learn more about this person and the incident itself. We might never know all the details, but without a doubt it will be a combination of different factors that go above and beyond just combat-related PTSD.

    4 deployments! Humans are not fixed like this, no amount or type of training can prepare a soldier for this many deployments and expect him or her to come away without permanent mental scars that will affect them the rest of ones lifetime.

      Now Robert Bales has a high-profile lawyer John Henry Browne to defend him. The latter wants to convince the public that his client had an exemplary record, a decorated soldier who was injured in Iraq, to his brain and to his body. Still he was sent back to war despite the health problems. Bales neighbours in Seattle didn't want to believe that he had committed these atrocities.

You see, in the civilized world, we let our children be children. They are never taught hate and lies, so when they are faced with your ultra violence, they do not handle things well–at times. We do not stone or chop the heads of anyone experiencing mental health issues. Killing those people was wrong. Noone wins when horrible things like that happen.

    Also, Fareed this may be a good study or mention to Dr. Gupta. His resounding insight into the human anatomy might help spread the word to our Military leaders to back off on more than 2 deployments per soldier, no matter what.

Like I said before, we have no right to be in Afghanistan but the right-wing politicians in Washington want this war to further their political careers plus big business interests want to make a killing by profiting financially. William Calley did the same thing in Mi Lai for the very same reason. They were frustrated because the local people wouldn't collaborate and help them achieve a military victory as many German soldiers in France did during WW2. In other words, they were quite angry!

    Bales represents a portion of the soldiers who commit suicide every year.

    And they are many.

    Instead of striking out like Bales did,they strike within and kill the life they alone own.

    At it's core,this is a brain injured soldier who should have never been redeployed a fourth time.

    There are thousands of soldiers on that same tipping point right now.

    Does anyone think the U.S.military brass will change their mode of operating?

    Not a chance.

    I am saddened this soldier fell apart and killed so many people.

    But soldiers are people and you can only push a person so far before he breaks.

    Congrats,U.S.Military Brass,you found Bales breaking point.

    This incident is just another in a list of damaging actions of the US military in Afghanistan. Soldiers have been deployed and re-deployed on a number of occasions which has resulted pressure on the physique of the individuals involved. America is going to have back down in the use of force to resolve issues if it wants to avoid incidents like this one!!

    I am horrified that this DOCTOR would dare write an op-ed piece on a patient he has never met. And who are you Dr. Harry Croft to state anything about a subject you haven't researched. A little education: PTSD is not the only possibility, look up "Post Concussion Syndrome" via the Mayo Clinic. It does not show up on MRI or MRA images, so it is often overlooked by neurologists. It occurs as a result of a medical history of multiple concussions, like from combat or sports (ie, hockey players, and etc.). As a result of Post Concussion Syndrome and returning to work too soon (a year after the injury), I loss my position at The Ohio State University Medical Center. Over a year later, I still exhibited personality changes. I was more prone to angery outbusts, belief that management was attempting to force me into making mistakes so as to undermine me professionally. I still have memory difficulty to include , but not limited to, approximately one-third of my life long memory are gone. I have never been able to tell my wife I don't even remember our wedding.
    So Dr. Harry Croft, maybe after you review the Mayo Clinic's research, you might wish to revisit this article and see if this soldier might be one more person over looked by an overworked neurologist and he should have never been returned to combat so soon.

    نشرت صحيفة ذي غارديان لائحة بأهم الشخصيات التي ورد ذكرها في ثلاثة آلاف رسالة إلكترونية حصلت عليها، والتي تقول الصحيفة إن جزءا كبيرا منها نسخ من البريد الإلكتروني للرئيس السوري بشار الأسد وتصف الصحيفة الشخصيات التي تبادلت الرسائل أو لها علاقة بهذا العنوان البريدي بأنها مجموعة صغيرة ولكنها متنفذة وهم كالتالي:

    بشار الأسد: يقول الناشطون إن الرئيس السوري بشار الأسد يستخدم اسم "سام" (Sam) كاسم حركي في مراسلاته عبر الإنترنت. وقد لاحظ الناشطون -الذين راقبوا هذا العنوان لشهور عديدة- أن صاحبه شديد الحذر ويتمتع بحس أمني عال، ويقوم بمسح الرسائل الصادرة سريعا.

    أسماء الأسد: زوجة الرئيس السوري، وتستخدم بريدا إلكترونيا تحت عنوان للمراسلة مع العنوان المذكور والحرفان الأولان يرمزان إلى عالية كيالي معلوف. وغالبا ما خاطب sam@alshahba.comصاحبة الحرفين AوK بعبارة "عزيزتي أسماء".

    ويعتقد الناشطون أن عالية كيالي الحقيقية ليس لديها حق الدخول إلى العنوان، ولكنهم لاحظوا أيضا أن صاحب هذا الحساب لا يتخذ إجراءات أمنية عالية مثل صاحب الحساب "سام". وتصف الصحيفة المراسلات بين العنوانين البريديين أعلاه، بأنها تنم عن شخصين يرتبطان بعلاقة زواج، ولكن علاقتهما ترزح تحت وطأة ضغط العمل الشديد، ولكن على أي حال فإن المراسلات بين الطرفين لا تنم عن اكتراث بالعنف الدموي الذي يجتاح سوريا.

    شهرزاد الجعفري: ابنة السفير السوري لدى الأمم المتحدة، وهي في بداية العشرينيات من عمرها. ويعتقد أن الجعفري وبعد تخرجها من الجامعة في الولايات المتحدة، عملت متدربة في مؤسسة براون لويد للعلاقات العامة في نيويورك بالولايات المتحدة. وبعد عودة الجعفري إلى دمشق، سرعان ما أصبحت من المقربين من الأسد وأعلى مستشاريه الإعلاميين.

    ويقول زملاؤها في مؤسسة براون إنها استمرت في التواصل معهم، واستخدمت علاقاتها مع المؤسسة لتحصل على ثقة الأسد كشخصية لها خبرة في مجال العلاقات العامة. إلا أن إحدى الرسائل التي تبادلتها الجعفري مع مسؤول رفيع المستوى في المؤسسة أظهرت دعمه للجعفري والأسد، ولكن المؤسسة بررت ذلك بأن كلمات المسؤول لم تكن سوى كلمات تشجيع لمتدربة سابقة في المؤسسة، وهي لا تعني دعما للأسد.

    هديل العلي: مستشارة صحفية إلى جانب الجعفري، وتتمتع بحظوة لدى الأسد وتأخذ على عاتقها كتابة تقارير عن الصحف اليومية وطريقة تعاملها مع الشأن السوري، ووقع خطابات الأسد على المناصرين، بالإضافة الى تأمين إيصال طلبات مقابلة الأسد من صحفيين يعتبرون مقبولين لدى النظام.

    لونة الشبل: مذيعة سابقة في قناة الجزيرة. تقدم دعما قويا للأسد وقد تكون من أشد المطالبين بانتهاج أسلوب متشدد إلى أكثر حد مع المعارضين السوريين. كما تقدم المشورة بشأن كيفية إبقاء الانتفاضة تحت السيطرة.

    خالد أحمد: أحد اللاعبين الأساسيين في المراسلات، ويقوم بتقديم تقييمات منتظمة عن الوضع المتردي في حمص. وهو مكلف بمهام إشرافية بوصفه مسؤولا سابقا في المنطقة التي تضم حمص. ويعتقد أن لأحمد صلات بالحزب القومي الاشتراكي السوري الذي ظل ومليشياته لمدة أربعين عاما من أشد المناصرين للنظام السوري. ويردد خالد رواية النظام أن هناك إرهابيين إسلاميين مدعومين من الخارج يحركون الانتفاضة السورية.

    حسين مرتضى: رئيس قناة العالم التلفزيونية الإيرانية، ويتمتع مرتضى بصلات قوية بإيران وحزب الله ويقدم النصائح والمشورة للأسد بشكل مستمر، ويدعي في مراسلاته مع "سام" بأن ما يقوله يمثل وجهة نظر إيران وحزب الله. ويعتبر مرتضى عنصرا رئيسيا في تسويق رواية النظام عن ما يجري في سوريا.

    According to a pilot study published in the latest issue of the peer-reviewed International Journal of Healing and Caring, veterans with high levels of PTSD saw their PTSD levels drop to within normal limits after treatment. They reported that combat memories that had previously haunted them, including graphic details of deaths, mutilations, and firefights, dropped in intensity to the point where they no longer resulted in flashbacks, nightmares, and other symptoms of PTSD. The study involved veterans from Vietnam, as well as more recent conflicts. ,
